​Fee for Service

Wound & Primary Care LLC does not participate in any insurance plans, our practice operates on a fee-for-service basis or membership fee.  Our fees are seasonable price, with monthly fee in certain cases being lower than co-pay or coinsurance fees and our yearly fee being below the majority of insurance deductibles.  ​

 

If you do have a health insurance plan, you can still elect to use your insurance elsewhere (specialists, pharmacy, etc.) just as you wound normally.  If you have questions about how health insurance is best structured with our services, we are happy to discuss it with you.

Membership

We provide memberships for a fee, which is not an insurance policy.  Membership serves as an enhancement, as we provide for most of your primary care needs, and insurance provides for catastrophic coverage.  We are an independent, alternative way to receive your wound care, weight management and primary health care, but not a complete substitute for health insurance.  We encourage, but don't require, our members to carry some type of health insurance policy in case of unexpected, expensive health events such as surgery or hospitalization.

 

Our simple, monthly membership fee covers the vast majority of wound and primary care services without insurance hassles.  We help you navigate beyond unnecessary testing, treatments, and prescriptions, guiding you toward a healthy lifestyle.  Our goal is to prioritize your medical freedom, by giving you honest and unbiased care.  We do not allow the insurance company, pharmaceutical companies or government agency to dictate your health care, but work with you to help you choose the care you want and need.  After all, should you not be free to make your own decision when it comes to your body.

Membership fees are:

Our membership fee(s) are started upon enrollment.  Future membership fees are paid by the calendar month on an ongoing basis until cancellation.  Payment of membership fees is required to remain an active member and receive service.  An initial one time fee plus a visit charge is applied to membership.  Then only membership fees are charged.

Memberships does not require Long Term Contracts or Commitments.  However, cancellation and re-enrollment may require an initial intake fee plus the visit fee to restart membership.

Addition cost after the 45 minutes.                                                                        $25 dollar/15 minutes

Supplies cost depends on requirement, not covered under membership.

Cancellation

You may cancel your membership at anytime for any reason by phone, message or mail.  All charges, including membership fees, will continue to accrue until notice of cancellation is given.

Re-Enrollment

If a member discontinues membership, by choice or lack of payment, the member may rejoin Wound & Primary Care LLC in the future, but will be required to pay an “Re-Enrollment” fee of $70 per member in addition to re-establish your membership fee.

Service Fees

All services not covered by membership fees (e.g. certain supply, non-covered labs, etc.) may be required to be Paid In-Full at the Time of Service.  These addition fees include wound debridement supplies, casting supplies, injections, or other supplies not covered by membership.  Labor is covered in your membership fees.  

 

Labs tests are provided at a minimal fee to members, and some basic test are free.  Labs not performed by membership fee can be completed at a LaboratoryAssist at low price or if available billed to your insurance.  ​​

Imaging studies at low fees are provided by Radiology Assist at the time of this website publication prices were as listed below.  If available imaging studies maybe covered by insurance.

Cancellation

All cancellations for scheduled visits must be done a minimum of 24 hours prior to scheduled time.  If failure to do so and this becomes a continuous problem a fee of $25 will be charged for each missed visit.

Telemedicine

Remote Patient Monitoring involves the collection and analysis of your physiologic data to develop and manage a treatment plan related to a chronic and/or acute health illness or condition.  On longer do you have to make multiple trips to the clinic, urgent care or emergency departments.  You can interact with your physician from the comfort of your home.  This will be included in your membership fee.  You will have to have an initial visit for a physical examine, which can be done in your home.

These are general payment guide lines and we can discuss any cost prior to any extra expense.
